As “ ENTO EAR BURUN THROAT ÖZEL SAĞLIK HİZMETLERİ A.Ş. “  (in short  “  ENTO KBB “ )  , as the Data Controller in accordance with the “Personal Data Protection Law” numbered 6698, we carry out the collection, purpose of use, processing, transfer, storage period and data privacy and protection activities of personal data within the scope of the Personal Data Protection Law, regulations and legislation. We take security measures within the scope of the Information Security and KVK Law and inform you about your rights, we attach great importance to the security of your personal data. We protect and store all kinds of personal data shared by our patients and their relatives in a way that will show care for patient privacy in accordance with the KVK Law and take all necessary technical and administrative measures to ensure the appropriate level of security for your personal data.

1. COLLECTION, PROCESSING AND PROCESSING PURPOSES OF PERSONAL DATA

“  ENTO KBB “    We process and store your personal data within the framework of the legal regulations and regulations and legislative provisions specified in the Personal Data Protection Law No. 6698. Your personal data is collected for the purpose of providing health services within the legal framework and  fulfilling the obligations arising from the contracts made by “  ENTO KBB “  and other legal obligations completely and completely. Personal data is collected verbally, in writing or electronically by automatic or non-automatic methods, call center, website, online services, WhatsApp and similar means, depending on the health services provided.

Your Personal Data;
Your name, surname, TR ID number, passport number or temporary TR ID number, place and date of birth, marital status, gender, patient file content created by our Medical Center and other identification data that we can use to identify you; your financial data such as your address, telephone number, e-mail address and other communication data, payment, credit card, bank information and billing information; audio and/or digital information obtained by us from time to time by electronic or non-electronic means; your data obtained and/or that may be obtained as a result of your visits to our Medical Center or Website ( www.entokbb.com  ), as well as your visual and/or audio camera recordings; your general and special personal data, especially personal health data obtained during the execution of all medical diagnosis, examination, treatment and care services; The necessary data, including but not limited to your Private Health Insurance data and Social Security Institution data, your health and identity data sent through various communication methods on our website, and your financial data such as payment and billing information, will be processed by our Medical Center.

Your personal data may also be processed when you participate in training, seminars, surveys or various organizations organized by “  ENTO KBB”  or when you visit or browse the internet pages.

Your personal data will be processed in accordance with the personal data processing conditions and purposes specified in Articles 5 and 6 of the Personal Data Protection Law. Your special personal data, especially your health data, and your general personal data may be processed by our Medical Center for the purposes specified below;

Your personal data, including special personal data;

• Fulfilling our legal obligations set forth in the Health Services Fundamental Law No. 3359, the Decree Law on the Organization and Duties of the Ministry of Health and its Affiliated Institutions, the Private Hospitals Regulation, the Regulation on the Processing and Ensuring Privacy of Personal Health Data and other relevant regulations,
• Protection of public health, provision of preventive medicine, medical diagnosis, treatment and care services,
• Health services and financing planning,
• Invoicing for our services,
• Sharing requested information with private insurance companies within the scope of financing health services,
• Ensuring financial agreement with our contracted institutions regarding the healthcare services provided to you;
• To be informed about the appointment if you make an appointment,
• Providing campaigns and information via message, e-mail or other means,
• Planning the internal functioning of the Medical Center,
• Monitoring and preventing abuse and/or unauthorized transactions,
• To answer all your questions and complaints regarding our health services,
• 
  Taking all necessary technical and administrative measures within the scope of data security of our Medical Center's systems and applications ,
• Analysing your use of healthcare services and storing your health data in order to develop and improve the healthcare services we offer to you, and preserving information regarding your health data that must be stored in accordance with the relevant legislation,
• It may be processed for similar purposes, including, but not limited to, measuring patient satisfaction.

In addition, your personal data may be processed within the framework of the Health Services Fundamental Law No. 3359, the Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Institutions, the Private Hospitals Law and Regulation, the Regulation on the Processing and Protection of Privacy of Personal Health Data, and the Ministry of Health regulations and other legislation provisions, and  may be transferred to the physical archives and information systems of our “  ENTO KBB”   Medical Center and/or our suppliers and kept in both digital and physical environments. In addition, the data of our Medical Center employees may be processed by us or real or legal persons we cooperate with or authorize, within the scope of the obligations stipulated by the Labor Law and Labor and Social Security Legislation and other legislation in force, for operational reasons such as increasing employee satisfaction and ensuring occupational safety and peace, and in accordance with the employer's right to management.

2. PERSONS AND ORGANIZATIONS TO WHICH YOUR PERSONAL DATA MAY BE TRANSFERRED

Your personal data will be processed within the scope of the Law and similar legislation and for the purposes stated above:

• Ministry of Health and its sub-units, Family Medicine Centers,
• Social Security Institution,
• Private insurance companies (health, retirement and life insurance, etc.)
• General Directorate of Security and similar law enforcement forces,
• Population Directorate,
• Turkish Pharmacists Association,
• Chamber of Turkish Physicians,
• Chief Public Prosecutors, Courts, arbitrators or referees in arbitration proceedings,
• Laboratories, centers and similar third parties with whom we cooperate for medical diagnosis,
• Your authorized representatives,
• The healthcare institution to which the patient is referred or to which the patient applies,
• Third parties from whom we receive advice, including lawyers, tax advisors and auditors,
• Regulatory and supervisory institutions, official authorities
• It may be transferred to our suppliers, support service providers, business partners and other third parties whose services we use or with whom we cooperate, within the framework of the personal data processing conditions and purposes specified in Articles 8 and 9 of the KVKK.

3. METHOD AND LEGAL REASON FOR COLLECTING PERSONAL DATA
   Your personal data is collected in all kinds of verbal, written or electronic media,  in accordance with the principles set forth in the KVKK, for the purposes set forth above and to provide health services within the determined legal framework and to fully and duly fulfill the contractual and legal obligations of “  ENTO KBB”  within this scope.
4. YOUR RIGHTS REGARDING THE PROTECTION OF PERSONAL DATA

Regarding your processed personal data, in accordance with the legislation;

• To learn whether personal data has been processed or not, and to request information about it if personal data has been processed,
• To access and request personal health data, to learn the purpose of processing personal data and whether they are used in accordance with their purpose,
• To know the third parties to whom personal data is transferred, either domestically or abroad,
• To request correction of personal data if it is processed incompletely or incorrectly, or to request deletion or destruction of personal data,
• To request that the personal data be corrected if it is processed incompletely or incorrectly and/or that the personal data be deleted or destroyed, and to request that the third parties to whom the personal data is transferred be notified,
• To object to a result that is to the detriment of the person himself/herself, as a result of the analysis of the processed data exclusively through automatic systems,
• In case of damage caused by the unlawful processing of personal data, you have the right to demand compensation for the damage.
