GENERAL EXPLANATIONS,

In the Law on the Protection of Personal Data numbered 6698 (“Law”), the personal data owners (“applicant”) who are identified as the relevant person are given the right to make certain requests regarding the processing of their personal data in Article 11 of the Law.

In accordance with the first paragraph of Article 13 of the Law; The data controller is “ENTO EAR NOSE AND THROAT PRIVATE HEALTH SERVICES TRADE Inc.” (hereinafter referred to as “ENTOKBB” for short). Applications for these rights must be submitted to us in writing or by other methods determined by the Personal Data Protection Board (“Board”).

C. DATA OWNER REQUESTS

As the data owner, please check the relevant box(s) in the list below for the situation(s) in which you wish to be informed under Articles 11 and 13 of the Law.

In the applications to be made by 3rd persons on behalf of the data owner, the notarized power of attorney together with this application form, and in the applications to be made on behalf of the children under custody/guardianship, a copy of the documents confirming the custody/guardianship relationship together with this application form must be sent to us.

In order to ensure the security of your personal data, within seven (7) days from the date of receipt of your application for information to the ENTO KBB, “ENTO KBB” will be able to contact you to confirm that you are the data owner and request some information and documents from you in this regard. The information and documents that you provide to us in this context will be destroyed immediately after confirming that you have the data.

In the event that the requested information and documents are missing, the information and documents will need to be completed and forwarded to us upon our request. The period of 30 (thirty) days specified in Article 13/2 of the Law on the conclusion of the request shall be suspended until the information and documents are fully communicated to us.

D. TERMINATION OF THE DATA SUBJECT'S REQUEST
